Cancer Patients
The American Cancer Society (ACS) offers additional reduced rates at various downtown and suburban hotels to cancer patients. For more information on the ACS's "Guest Room Program," please contact Mary Herbert or Patricia Parker at (877) 824-0660 or visit the Cancer Resource Center. If you are a physician or social worker, please call (773) 702-9900 or fax your requests to (773) 834-0261. The request should include the patient's name, address and home phone number; type of cancer; doctor's name and phone number; and dates of treatment. The patient must also have a signed patient waiver form on file. This form will be faxed to the doctor's office once the housing request has been completed.
